Gaps surface as denials
Without a plan, coverage changes show up weeks later as denied claims, rework, and aged A/R — not at registration.
Medicaid 2027 | Revenue Protection
Starting January 1, 2027, many Medicaid expansion adults face work requirements, six-month renewals, and shorter retroactive coverage. tevixMD goes beyond the eligibility check to find missing insurance, validate identity, and surface potentially billable coverage.

The bigger problem
An eligibility check answers one question:
Is this coverage active? → No.
Your team still needs these answered.
Does the patient have replacement coverage?
Did coverage transition to a commercial plan?
Is a Medicare or Medicaid identifier missing?
Is incorrect demographic data preventing a match?
Are coordination-of-benefits indicators available?
Is there another payer worth investigating?
Knowing coverage is inactive is information. Knowing what to do next is intelligence.
Coming 2027 · Medicaid Coverage Monitor
With only one to two months of retroactive coverage, finding out at billing is often too late. Coverage Monitor looks for the patient's current coverage automatically.

Every client · at patient arrival and in batch
When an eligibility check returns inactive Medicaid, insurance discovery runs on its own. At arrival, the front desk sees the result with no extra step. In batch, the output file includes any coverage found.
Patients who come back often — dialysis, behavioral health, therapy
No appointment data needed — your own verification history defines who is recurring, so you only pay to monitor patients likely to return.
Clients who share their schedule
Send upcoming appointments and we check each Medicaid patient ahead of the visit. Three to five days gives you time to call the patient or line up other coverage. Earlier is riskier, because Medicaid coverage usually ends at month-end.

Why getting ahead matters
—
days to January 1, 2027
States may receive limited extensions, but you can't plan your revenue around one.
July 2025
H.R. 1 signed, setting new Medicaid eligibility rules for certain adults.
Now
Time to baseline coverage gaps, connect systems, and define inactive-response workflows.
Jan 1, 2027
Community engagement requirements and shorter retroactive coverage start for affected groups, unless a state receives an extension.
Mid-2027
First six-month renewals for affected expansion adults. More patients lose coverage between visits.
Dec 31, 2028
Latest date a state can delay the work requirement under a good-faith extension.
The cost of waiting
Without a plan, coverage changes show up weeks later as denied claims, rework, and aged A/R — not at registration.
The cost of waiting
With one- to two-month retroactive coverage, the window to fix a gap after the visit is much shorter.
The cost of waiting
Integrations, batch files, and staff workflows need to be live before volume rises, not after.

Investigate missing, unknown, or outdated insurance coverage using available patient information.
Help identify available government and replacement coverage, including supported Medicare Beneficiary Identifier (MBI) workflows.
Improve the accuracy of patient information used in eligibility searches and downstream revenue cycle processes.
Surface available coverage and COB indicators to support payer review and billing decisions.
Support verification and discovery through available user-interface, API, and batch processing options.
Capability availability varies by subscription, payer connectivity, and deployment configuration.
Better information starts with better identification
Patients don't always arrive with accurate insurance information. Supported tevixMD workflows can start a coverage search from a phone number, before the account defaults to self-pay.
Results depend on available data, identity matching, payer sources, and supported workflows. A phone number alone does not guarantee a coverage match.

What changes in 2027
The new federal rules apply mainly to adults in the Medicaid expansion group, not every enrollee. For providers, coverage verified at one visit may not hold at the next.
Certain Medicaid expansion adults must show 80 hours per month of work or qualifying activity, subject to statutory exemptions. States must implement by January 1, 2027, with limited good-faith extensions possible.
Affected expansion adults move from annual to six-month eligibility renewals, beginning with renewals scheduled after December 31, 2026.
Retroactive coverage narrows from three months to one month for expansion adults and two months for other eligibility groups, beginning with applications on or after January 1, 2027.
Coverage has to be right on the date of service, not just at registration.
Applicability varies by eligibility category, exemption, state implementation, and effective date. States may receive limited good-faith extensions for the community engagement requirement. This summary is informational, not legal advice. Last updated October 13, 2026.

FAQ
tevixMD is a technology provider. It does not determine Medicaid eligibility, check work-requirement compliance, or renew coverage. Last updated October 13, 2026.
Federal legislation enacted in 2025 (H.R. 1, Public Law 119-21) introduces community engagement (work) requirements for certain expansion adults, six-month eligibility redeterminations for expansion adults, and shorter retroactive coverage periods. States must implement the community engagement requirement by January 1, 2027, though CMS may grant limited good-faith extensions. Exact timing and operations vary by state.
Not all Medicaid enrollees. The work requirement and six-month renewals apply primarily to adults ages 19–64 in the ACA expansion group, subject to exemptions such as pregnancy, certain disabilities and medical frailty, caregivers of young children, and others defined in statute. Children, most older adults, and many people with disabilities are not subject to the work requirement. Retroactive coverage changes apply more broadly, with different limits for expansion and non-expansion groups.
Best practice is to verify eligibility for each date of service, not just at first registration. With more frequent renewals for affected adults, coverage can change between visits, so recurring patients and scheduled services warrant re-verification close to the encounter.
Treat it as the start of an investigation rather than the end. Confirm the patient's identity and demographics are correct, check whether the response reflects a data mismatch, and search for other potential coverage such as commercial, Marketplace, Medicare, or a different Medicaid plan before routing the account to self-pay.
Yes. Some patients transition to employer coverage, Marketplace plans, Medicare, or other programs. Others may have had secondary coverage all along. Whether coverage exists — and whether it applies to a given date of service — must be confirmed with the payer.
Insurance discovery can help locate coverage that isn't in the patient record, based on available data, identity matching, and payer sources. It cannot guarantee that replacement coverage exists or will be found, and identified coverage should be verified for the date of service before billing.
Sometimes. Eligibility responses vary by state Medicaid agency and managed care plan. Some return eligibility begin and end dates or plan-level detail; others do not include renewal or redetermination dates. Providers should not rely on a single response to predict future coverage.
tevixMD supports verification and discovery through available user-interface, API, and batch options. The right approach depends on your volume, systems, and use cases, and specific capabilities can vary by deployment and agreement — we'll scope this with your team.
No. tevixMD is designed to work alongside your existing EHR, practice management, billing, and clearinghouse systems, adding verification, discovery, and validation capabilities to your current revenue cycle workflows.
Identify which of your patient populations are likely affected, move eligibility checks closer to each date of service, define a clear workflow for inactive responses, improve demographic data quality at intake, and evaluate how coverage discovery fits your operation. Monitor your state Medicaid agency's implementation timeline.
